As with any technology the most common cause of problems is human error. This principle applies to access control as much as anything, and in this case it is usually someone finding themselves locked out because they forgot their card/key/token.
While much as been done to try to remedy this, small tokens that hook on your keys or belt and work via proximity sensors as an example, there is still the chance that the person who is authorised to open a door can not because they can't prove who they are.
The other problem is that access control tokens can fall into the wrong hands. While it is a simple matter to void a lost, stolen, or otherwise void token with a modern access control system, this relies on staff being aware and willing to admit that their token is lost or stolen.
